Please use the MyMedicalLocker portal to update your information and digitally sign any forms required before being seen by a physician in the office.

Day of your Visit

When you check in you will also be able to access and update your information on our in-office tablets for every visit to provide the most complete up-to-date medical record.

To expedite your check-in time, any information that can be filled out and updated prior to arrival will save you time with the check in process. We suggest you still plan to arrive 15 min prior to your appointment time to update your information at every visit.

Upon arrival for your appointment we will request information from you regarding your:

• reason for visit
• medical history
• insurance provider
• primary care physician
• preferred pharmacy

Please bring your driver's license or government issued photo ID and current insurance card.

Please plan to arrive 15 minutes prior to your appointment in order to register and check-in.
